The shot was taken with a D70 with Nikkor AF Micro 60mm f/2.8D. I have an old Sunpak Gx14 flash (guide number ~22m @ ISO200) mounted on a DIY bracket so that I can position it just above the lens. The front of the flash has a piece of paper taped over it to diffuse the light slightly.

No trouble at all. It is a photography forum and "a picture paints a thousand words" so it was actually easier to show a picture than write it up.
